To insure that each patient receives prompt attention, we provide:
- Referral services 24 hours a day, 7 days a week
- 24 hour telephone support
At the start of home care services a registered nurse will come to the home to evaluate the patient's medical care needs. The nurse will then design a treatment plan based on the evaluation and orders from the patient’s physician. In addition, the nurse will:
- help patients and families learn the skills needed to manage at home
- work closely with the patients physician to make sure all medical needs are met
- tailor home health services to the unique needs of each patient
- help the patient manage medications, chronic conditions and/or assist with rehabilitation
- coordinate with other community agencies for other services or equipment the patient might need at home
Payment for Home Health VNA services
The cost of care provided by Home Health VNA is typically covered by health insurance including most private insurances, Medicare and Medicaid and Senior HMO’s. Patients receiving home care services must meet certain requirements including:
- Services must be ordered by the patient's doctor
- The patient must be “homebound” or unable to leave home without great effort
- The patient’s condition requires skill care by nurse or therapist

Experience has shown that early referral for hospice care is most beneficial to the patient both physically and emotionally. The decision to choose hospice belongs to the patient. In general, it is time for hospice when:
- The patient and family need help and counseling to adjust to the diagnosis.
- The family would benefit from assistance at home caring for the patient and time to prepare for the future.

Payment for Hospice care
Hospice care, including all medical equipment, supplies and medications related to the hospice diagnosis are typically covered under private health insurance, Medicare, Medicaid, and Senior HMO’s. Private Care Services of Home Care, Inc.
Private care services begin with an initial meeting at the home, hospital, rehabilitation center or nursing home. During this meeting our nurse will discuss all the services available and then design a customized plan of care to meet the needs of the patient and family.
Private care services are available on an hourly bases to 24 hours a day 7 days a week. All staff that provides personal care services are fully licensed and/or certified and have had a complete background check. In addition they are supervised by a licensed nursing professional to insure optimum service quality. Services can begin quickly and easily with one phone call. Please call .
