Home Health VNA Services

Nursing Services are provided by registered nurses and licensed practical nurses. Nurses work in conjunction with a patient’s physician. The nurse will come to the home to evaluate the patient’s needs and develop a plan of care. Through regularly scheduled visits to the home, the nurse will attend to the patient's medical needs, provide teaching and support and coordinate any additional services the patient may need. For patients with more critical needs, advanced nursing services include:
- Cardiac and Respiratory Treatment

- Pain and Symptom Management

Orthopedic & Rehabilitation Services are provided by registered therapists and are available to meet specific therapy needs to improve movement, to restore skills needed for daily living and to regain skills in speech and learning. Services include:
- Occupational Therapy
- Physical Therapy
- Speech Therapy

IV Therapy Services are provided by expertly trained registered nurses and are designed to meet the needs of patients requiring complex IV treatment. IV therapy can include:
- antibiotic treatment
- chemotherapy
- fluids for hydration
- pain management medications

Mother and child care services are provided by registered nurses and include nursing care for the mother and child, parenting education and specialized care for infants and children with special needs. Mother and child services can include:
- high risk prenatal care
- post partum or after delivery nursing
- home phototherapy treatments for jaundice in newborns

Pediatric Services includes infant, child and adolescent care for patients with chronic and acute complex illnesses or conditions including pediatric palliative care and cancer treatment. Services can include:
- highly technical nursing services
- family teaching

Nutrition Counseling is provided by a Registered Dietitian or a Diabetic Educator. Counseling can include recommendations on a diet based on a patient’s lifestyle and medical needs, information on dietary guidelines for a medically prescribed diet, and techniques for menu planning, shopping and cooking.

Mental health services are provided by licensed social workers and are available to help improve or maintain the patient’s social, emotional and physical health. Mental Health services can include:
- Counseling by a social worker
- Psychiatric nursing consultation, evaluation, education and intervention.

Home Care Aides provide assistance with personal care such as dressing and bathing, light meal preparation, housekeeping and errands. All services are provided by trained or certified aides or homemakers who work under the supervision of a registered nurse.
Merrimack Valley Hospice Services

Nursing Care is provided by registered nurses who act as the patient’s care manager making regular visits to the patient’s home to monitor the patient and provide expert pain and symptom management.

Social Work Services are available for patients and families who would benefit from emotional support and counseling. Social Workers can assist the entire family with the day to day concerns that often come when a patient is facing the end of life.

Home Care Aides are certified nursing assistants who provide expert personal care including dressing, bathing and grooming to insure the highest level of patient comfort.

Spiritual Care is provided by chaplains who offer non denominational support to patients and families and can act as a liaison with the patient’s own religious community.

Expressive Therapy in hospice care includes services that would be of interest and benefit to the patient including music, texture, art, sand therapy, message, journaling and Reiki.

Volunteers, specially trained in the needs of hospice patients and their families, can provide a variety of services to enhance and support care. Bereavement Support is counseling and support services totally focused on the needs of the caregivers and family. Services can include individual and group counseling as well as community based support groups for people of all ages. Click for a list of upcoming support groups. Private Care Services

Personal Care includes assistance with dressing, bathing and grooming provided with the utmost care and respect.

Private Care Nursing can include a wide variety of services including IV therapy, dressing changes and medication administration. Our nurses can also teach patients about a specific illness or injury and answer questions.

Light Housekeeping can include laundry and ironing, meal clean-up and basic housekeeping chores such as dusting and vacuuming.

Meal preparation to insure proper nutrition and diet.

Medication reminders to insure that medications are taken correctly.

Dietary Consultation for patients with a medical condition that requires a special diet.

Companionship can include help with writing letters, answering correspondence, engage in in-home activities such as board games or cards.

Caregiver Respite/Relief is available for family members or other caregivers who are temporarily unable to care for a family member.

Transportation/Errands to take the worry out of routine travel to the doctor’s office or other routine errands.
