Special Programs

• The Hand in Hand Dementia Program is a specialized program designed to meet the complex needs of patients who have dementia. Services are provided in homes, nursing homes, assisted living residences and hospitals. Click to download a brochure.
• Pediatric Palliative Care Program is designed for the special needs of children with life-threatening illness and their families. All services are in addition to those provided by the child’s doctor and are designed to improve the quality of life for the child and family.
